As a provider, we strongly recommend that you choose to complete your intake meeting with your Benefits Counselor either by phone or video meeting, however we understand that many customers have accessibility needs, including language and communication barriers, and would prefer a digital format. With our new Virtual Intake Meeting, you now have an additional choice to meet your unique needs and preferences!
The below form is only for current Vocational Rehabilitation Customers who have been referred to Benefits Consulting Services for WIPA Benefits Planning Services
*For security purposes, the Virtual Intake Meeting does not ask any Personally Identifying Information (PID). All transmissions are secure, and all questions are voluntary.
*For accessibility purposes, this virtual intake is available in large-font formats, screen-reader accessible, and can be translated into multiple languages.
